Who is not a candidate for breast augmentation?

Breast augmentation has a high patient satisfaction rate and is appropriate for healthy individuals with appropriate expectations. However, breast augmentation is not appropriate for individuals that have any of the following:

  • Body Dysmorphic Disorder or other unstable psychiatric illnesses
  • expectations of salvaging a relationship by undergoing breast augmentation
  • peer pressure from friends, co-workers, family members or spouses
  • collagen vascular disease
  • age less than 18 years
  • severe fibrocystic changes or atypia, as breast augmentation can make mammography more difficult
  • unrealistic expectations

Which implant type is best for me?

The choice of implant can be confusing for individuals seeking breast augmentation, and the decision should be based on your desired look and feel following the procedure. Your surgeon will guide you in this process during preoperative consultations.

Implants can be saline-filled or silicone-filled, smooth or textured, round or anatomically shaped.

Silicone gels tend to have a feel that more closely resembles that of natural breast tissue. Silicone gel breast implants are FDA approved for use in individuals 22 years of age or older. Therefore, silicone gel implants are not for use by individuals under the age of 22.

Saline implants are FDA approved for breast augmentation in individuals over the age of 18. Saline provides a firm feel and look, which some individuals find desirable. Incisions for breast augmentation can be shorter when using saline implants because the implant can be placed before filling them with saline, which is not an option with silicone implants.

The choice between textured vs. smooth implants will largely depend on implant pocket selection. Textured implants are recommended for placing in a pocket directly under the breast tissue or above the muscle, as there is a decreased risk of capsular contracture with textured implants in this location. Smooth implants are preferable when placed in a submuscular pocket or under the muscle, and they tend to have a softer feel.

The final choice to be discussed when considering breast implant choices is shaped vs. round implants. Anatomically shaped implants are fuller in the inferior portion of the implant and are tapered towards the top. Rotation of this style of implant within the pocket can produce an unnatural and distorted appearance. Round implants, which are uniform in shape and size, can rotate without producing these undesirable changes in shape.

Can you tell me more about recovery the period following breast augmentation?

Breast augmentation is well tolerated by the vast majority of patients, and most individuals will go home the same day. Pain is mild to moderate and is well controlled by oral medications. For those who have the implant placed under the muscle, a muscle relaxant will be prescribed along with pain medication. In addition, medications for nausea, which is common after anesthesia, and an antibiotic will be prescribed for a few days following breast augmentation.

Strenuous activity and heavy lifting should be avoided following breast augmentation. You will also be asked to wear a supportive bra during this time period, which will both help control swelling and support the breasts as they heal.

At your first follow-up appointment, we will review implant manipulation techniques that will assist in keeping the pocket open and prevent scarring of the tissue to the implant and potentially distorting the shape of the breast. While simple and not time-consuming, you will need to perform these implant manipulation techniques for an extended period of time.

What results are expected from breast augmentation surgery?*

Immediately following breast augmentation, your breasts will have visibly increased in size. The degree of size change will be dependent on your individual preoperative goals and desired look as well as anatomical constraints encountered during your operation. My goal with breast augmentation is to give your breasts the fullness and shape that you desire while maintaining the natural, aesthetically pleasing appearance of your breasts and balance to your body proportions.

Long-term results may vary. Implants may need to be replaced, and changes may occur in the breasts due to normal aging, fluctuations in weight, pregnancy, and other factors. These changes may cause dissatisfaction in the appearance of your breasts, and restoration of a more youthful breast contour may require implant replacement and/or mastopexy, which is commonly referred to as a breast lift.

Is Breast Augmentation safe?

Breast augmentation is one of the most common procedures performed by plastic surgeons. During your consultation, Dr. Walker will perform a detailed history and physical exam. We will identify your breast augmentation goals, and together we will formulate a plan to help you achieve your desired breast appearance. We will also discuss the potential risks with breast augmentation surgery, which include the following:

  • unhappiness with size (too small or too large)
  • capsular contracture
  • distortion of breast shape
  • implant rupture or leakage
  • wrinkling/rippling of skin overlying implant
  • bleeding or hematoma formation
  • change in nipple sensation
  • infection
  • unfavorable scarring
  • blood clots
  • anesthesia risks
  • asymmetry in size or shape of breasts
  • seroma formation
  • potential need for further surgery

During recent years, there has been a growing concern of breast implants causing systemic disease and breast cancer. It is important to understand that breast implants have not been shown to cause systemic disease. More recently, there has been concern surrounding a potential link between breast implants and anaplastic large cell lymphoma (ALCL), which appears to arise in the scar tissue around the implant. There have been approximately 60 cases of ALCL reported in the millions of individuals who have undergone breast augmentation. Due to a lack of data showing a definitive link between breast augmentation and ALCL along with the extremely rare nature of this disease, I feel that breast augmentation is still a very safe operation to perform.

During the preoperative consultation, these potential risks will be discussed with you in detail. Should you have any questions or concerns, they will be addressed.
